Here’s what actually happens: when lemon is metabolised in the body, it produces alkaline byproducts — primarily citrate — that raise the pH of urine and blood. This is the basis of the claim that lemon water ‘alkalises the body.’

But — and this is the critical point — this alkalising effect happens after digestion, not in the stomach itself. In the stomach, lemon is still acidic. For someone whose reflux is triggered by direct acid irritation of the esophagus, lemon water adds more acid to an already acid-irritated system.
So the question is not ‘is lemon alkalising?’ — the answer to that is complicated and context-dependent. The real question is: does your specific digestive pattern benefit from the stimulating, sour quality of lemon — or does it aggravate it?
